Integrated digital care platform offered to healthcare providers to help citizens manage their chronic conditions from home during pandemic
ProACT is a cloud-based digital health technology that helps coordinate and support citizens to manage their chronic diseases (including those with multimorbidity) at home, supported by their care network.
The system links new and existing care applications, sensors and healthcare technology devices, providing a customised interface through tablets, smartphones and computers for patients, carers and healthcare professionals.
The platform has been designed to support the most vulnerable individuals through COVID-19, and allows patients to self-monitor on biometrics, integrated with critical COVID-19 risk factors.
ProACT has been developed and tested at a proof of concept level with 140 older adults (over 65) via an EU funded Horizon 2020 (peer-review successfully completed in January 2020), across three EU member states.
The company asks that basic costs are covered by the user to host the system (including paying for the ProACT citizen tool kits).
